Since 2019 Alpha Case Management and Occupational Therapy has been providing assessment, rehabilitation services and expertise in case management for individuals with acquired disability.
Functional Assessment
We provide in home functional assessment of clientele with acquired brain and spinal cord injuries. Assessments include Activities of Daily Living (ADL), Mobility, Care Needs, Community Access, Minor Home Modifications, Professional Reports, Establishing Treatment and Rehab Goals, Supporting individuals to maximise participation and independence in all aspects of their lives.
Case Management
We provide comprehensive, person centred, timely and accountable case management services to ensure the support of the treatment, rehabilitation needs of people with acquired injuries. Case management services are focused on attaining the optimal outcome for our clients. We manage and coordinate referrals to support and rehabilitation services, clear goal setting and outcome monitoring. We also provide education to clients and family to equip them with the skills and resources needed to self-manage their ongoing injury related needs.
Allied Health Services
Our diverse, multidisciplinary team is equipped to support the rehabilitation goals of our clients. We also work in partnership with established, experienced allied health providers and we are able to identify and refer to the most suitable provider to meet each individual’s rehabilitation goals.
